Overview

[99mTc]Tc-ZHER2:4107 is a technetium-99m-labeled second-generation affibody molecule designed for the diagnostic imaging of HER2 expression in breast cancer patients using SPECT/CT. Developed by the Tomsk National Research Medical Center, it utilizes a small (6.5 kDa) non-immunoglobulin affinity protein scaffold (ZHER2:4107) engineered to bind with high affinity to the human epidermal growth factor receptor 2 (HER2). Unlike therapeutic HER2-targeted agents, this radiopharmaceutical is used as a diagnostic tool to visualize and quantify HER2 levels in primary tumors and metastases. It is particularly effective in discriminating between HER2-positive and HER2-low/negative expression, which is critical for selecting appropriate HER2-targeted therapies.
